The tourist-visa limb is broadly workable, with one refinement: the visa you actually want is usually the S2. The transit limb is the problem — and it is the part most people get wrong, starting with the fact that the 144-hour scheme no longer exists.

1. The 144-Hour Scheme Is Gone — It Is 240 Hours Now
In December 2024, China's National Immigration Administration replaced the 144-hour transit visa-free policy with a 240-hour (10-day) one. As issued, the scheme covered 54 countries and 60 ports across 24 provinces; later announcements have since expanded it to 57 countries and 65 ports. The officially listed activities are tourism, business, visits and family visits.[4]
So if you are planning around '144 hours', you are planning around a policy that no longer exists. The analysis below concerns the 240-hour scheme — but the conclusion would have been the same under the old one, because what matters is not the number of hours. It is what the scheme is for.
2. A Transit Facility Is Not a Visa Substitute
The transit scheme is exactly what its name says: a facility for passengers passing through China. Two conditions define it. First, you must hold a confirmed onward ticket to a third country or region, departing within the permitted period — no onward ticket, no entry. Second, your activities in China must stay within the listed purposes. Medical treatment is not listed.[4]
This is not a technicality. Under the Exit and Entry Administration Law, a foreigner staying in China may not engage in activities inconsistent with the purpose of stay.[3] A pre-planned course of hospital treatment is the very purpose of the trip, not an incidental event during a transit — it sits outside the scheme's purposes from the start.
The same is true of China's unilateral visa-free entry (30 days for listed nationalities, extended through 31 December 2026): its listed purposes are business, tourism, visiting family and friends, exchange visits and transit — again, no medical treatment.[5]
3. The Risks of Using It Anyway
• Refusal at the border. Entry under the scheme is decided at border inspection. Where the officer can see that the trip is not a transit and the real purpose is not covered, entry can be refused — leaving you to rearrange onward travel on the spot, with a hospital appointment missed.
• A hard clock. Even if you are admitted, the permitted stay runs out in at most ten days, and treatment does not always keep to schedule. Staying beyond the period is unlawful stay: it can draw a warning or a fine — detention in serious cases — and it leaves a record that can complicate future visa applications.[3]
• No certainty in advance. A visa is applied for beforehand, on the stated purpose, with the hospital's documents in the file. The transit scheme offers none of that: the decision is made at the airport, after you have already travelled.
4. If Treatment Overruns: The Stay-Permit Remedy
The law does provide a remedy, but it works only if it is used in time. A foreigner who entered visa-free and genuinely needs to stay beyond the permitted period — for example because a course of treatment cannot be interrupted — may apply to the exit-entry administration of the local public security bureau for a stay permit (停留证件), supported by the hospital's documentation.[3][6]
Where a longer stay is needed, a residence permit for private affairs may be the appropriate instrument, again supported by hospital documentation. Either way, the application must be made before the permitted period expires. An application filed in time is a procedure; an explanation offered after an overstay is a much weaker position.[3][6]
5. The Proper Route: A Visa, Usually S2
There is no dedicated 'medical visa' in China's visa categories, and none is needed.[1] Short-term medical treatment is legally a private affair. State it honestly on the application, attach the Chinese hospital's appointment or invitation letter, and the consulate will issue the appropriate category — usually the S2 (short-term private affairs), sometimes the L (tourism).[2]
In practice, many foreign patients also receive treatment while in China on an L visa without any issue: receiving medical care is not an activity inconsistent with a lawful stay. What matters is entering on a basis applied for and granted in advance — not on a transit facility whose conditions your trip does not meet.[3]
Bring your medical records and the hospital's appointment letter, declare 'medical treatment' truthfully, and take whatever category the consulate issues. Keep the hospital documentation with you during the stay: it is also what supports a stay-permit application if treatment runs long.

In short: come to China for treatment on a visa, not on a transit facility. The 240-hour scheme is built for passing through — a confirmed onward ticket, a closed list of purposes, a hard deadline. A planned medical trip needs the opposite: a purpose declared in advance (usually an S2 visa), hospital documents in the file, and, if treatment overruns, a stay-permit application filed before your permitted stay expires. 二、过境便利不是签证的替代品

三、硬用过境免签的风险
• 在边检被拒绝入境。是否准予入境由边检在入境时决定。边检一旦看出这趟行程不是过境、真实目的又不在政策范围内,可以拒绝入境——你只能当场改签后续行程,医院的预约也随之落空。
• 时钟是硬的。即便获准入境,许可停留最长只有十天,而治疗不总能按计划表走。超过期限继续停留构成非法居留:可被警告、罚款,情节严重的还可拘留,并留下记录,影响今后的签证申请。[3]
• 事先没有任何确定性。签证是提前申请的:目的写明、医院材料在卷。过境免签给不了这种确定性——决定在你落地之后、在机场当场作出。
四、治疗超时怎么办:停留证件这条救济路
法律留了救济路径,但前提是及时用。免签入境的外国人确需超过免签期限停留的——例如治疗无法中断——可凭医院出具的材料,向当地公安机关出入境管理机构申请停留证件。[3][6]
如需停留更久,可申请私人事务类居留证件,同样以医院材料为支撑。无论哪种,申请都必须在许可停留期届满之前提出:按期申请是程序问题;逾期之后再解释,处境要被动得多。[3][6]
五、正确路径:办签证,通常是 S2
中国的签证种类里没有专门的“医疗签”,也不需要有。[1]短期就医在法律上属于私人事务。在申请表上如实填写,附上中国医院出具的预约或邀请函,领事馆会签发相应种类——通常是 S2 字(短期私人事务)签证,有时是 L 字(旅游)签证。[2]
实务中,不少外国患者持 L 签在华接受治疗也并无问题:接受医疗本身并不是与停留事由不相符的活动。关键在于,入境依据是事先申请并获批准的,而不是一项你的行程根本不满足其条件的过境便利。[3]
带好病历资料和医院的预约函,如实申报“医疗”,领事馆签什么种类就持什么种类入境。医院材料在停留期间也请留存好:万一治疗超时,它正是申请停留证件的支撑材料。
